The goal is to prevent and relieve suffering, to improve quality of life for individuals experiencing a serious, complex illness at the end of life.Your Primary Care Physician and a Hospice Physician agree that the Hospice criteria for a terminal illness is present, usually with a prognosis of six months or less. Our Hospice team will perform a specialized, individualized assessment taking into consideration any specific care needs and tailor a care plan centering on personal choices and needs.
