Outpatient Facility

Medicare-Certified Hospice Care

The first and most important aspect you should check for in quality hospice care is Medicare certification. To meet Medicare requirements, a hospice must provide specified core and auxiliary services. Core services include:

• Physician services
• Nursing services, available routinely or on call 24/7
• Medical social services by a qualified social worker
• Counseling, including but not limited to bereavement, dietary, and spiritual counseling

Auxiliary services include:

• Physical and occupational therapy
• Speech-language pathology services
• Hospice aide services
• Homemaker services
• Volunteers
• Medical appliances and supplies, including necessary medications
• Short-term inpatient care in a Medicare or Medicaid-participating facility
• Continuous home care for emergency situations

Outpatient Hospice Facility

La Bella Vita provides outpatient hospice care. Our staff takes care of hospice patients in their own homes, which include houses, apartments, nursing homes, or assisted living facilities. Our facility does not house patients for long-term care. If a patient requires inpatient care, we will refer them to a certified inpatient hospice facility.

Proximity to Your Home

We are located in Westlake Village, near Thousand Oaks past the west end of the San Fernando Valley and north of Malibu. For outpatient hospice care, proximity to your home is important. If you need to call on a nurse for an emergency, response time will be less if you live near the facility.
