La Bella Vita Hospice offers the comfort and convenience of hospice care in your own home. We are an outpatient facility, so we do not have an office location with medical amenities. Instead, we arrange for hospice nurses and physicians to administer care at the location of your choosing. This allows you or your loved one to remain in familiar, comfortable surroundings.
Patients facing serious illnesses often find it more beneficial to remain in their own homes. If a person has chosen hospice home care, they have chosen not to seek further treatment for their condition, rendering extended hospital stays mostly unnecessary. Our care manager visits or connects with you to assess your needs and help answer any questions you have.
Each patient and family will be connected with a hospice care nurse who can be reached by mobile phone during their service shift.
A scheduled nurse and caretaker is available on-call to assist with any needs to better cope with the spiritual topics that may be presented during end-of-life care.
A medical physician is constantly available to help provide medical services as needed. Our hospice services are designed to place patients in an environment with consistent, dignified care.
Our bereavement coordinator is available to provide consistent support to families and friends of patients during end-of-life care.
Nursing professionals, medical care assistants, volunteer companions can assist in the emotional support of families during hospice care.
